Description
Hand-N-Hand Outreach offers a program strictly for men who were just released from jail and are on parole. This sober living environment provides therapy and skill groups. Additional services include:
-Assessment and Screening
Health care referrals
-Assistance with social security benefits
-Assistance with Department of Public Aid
-Assistance with finding affordable living arrangements
-Counseling referrals
-Inpatient referrals and placement
-Ongoing case management services
-Legal assistance
-Educational referrals and placement
-Employment referral and placement
-Transportation assistance
-House meetings and activities

Eligibility
-This program follows State of Illinois Department of Alcohol and Substance Abuse guidelines.
-Individuals with criminal backgrounds are accepted on a case by case basis.
-All residents are asked to contribute toward their stay; how much each may pay is determined on a case by case basis.

Referrals are accepted from DHS, DCFS, or other service providers in the area. Call 773-722-7148 weekdays between 10:00am and 5:00pm to begin the intake process, or for more information.
