Description

Eva Mae Recovery Hope provides methadone maintenance for adults with opioid use disorder. Dosing is 6 days per week. Clients also receive group counseling, and are strongly encouraged to seek individual and family counseling as well.

Eligibility

-Eva Mae accepts Medicaid and Medicaid managed care plans, including Aetna, Blue Cross Blue Shield, IlliniCare, Meridian, and Molina.
-Self pay clients are accepted. The fee for self-pay clients is $40/week. There is also a $30 intake fee, for a total of $70 due at intake.
-Adults who have a substance use disorder other than opioid use disorder may receive group therapy only.

How to Apply

Call 773-952-6861 during office hours for an eligibility screening and to schedule an intake assessment. A determination will be made at intake as to the level of dosing and the number of counseling sessions you will be required to attend. Please bring the following to your appointment:

-Photo ID
-Second form of ID (e.g., Social Security card, passport)
-Medicaid card, if applicable
-Payment, if not covered by Medicaid

New clients can generally be admitted within a week.
