Association House offers mental health and substance use prevention, education, and treatment programs for youth and adults. Mental Health services include:

-Child, Adolescent, and Adult Counseling
-Family Counseling
-Case Management
-Pre-Admission Screening
-Assertive Community Treatment (ACT)
-Integrated Health Program
-24-hour Supervised Residential (placement by referral only)
-Psycho-Social Rehabilitation
-Community Support Team
-Mental Health Juvenile Justice

How to Apply

Walk in during business hours or call 773-772-8009.for intake. You must provide the following:

-Photo ID
-Insurance information (Medical Card of Medicare)
-Proof of income ( if no income, must go to local SSA office for printout to verify).

There is generally a wait list for counseling. Some services are available by referral only.
