Heartland Health Outreach, the health care arm of Heartland Alliance, is an established leader in providing affordable, high-quality health care to men, women, and children who would otherwise struggle to address basic health needs. Services include:
- Primary care
- Mental health
- Oral health
- Substance use treatment
Services are also available to low-income or homeless immigrant and refugee populations throughout the Chicago region.
Patients have access to healthcare support on a 24 hour basis, with emergency on-call and via the patient portal on the electronic health record. Other services include referrals to specialty and urgent care, optometry, mammograms and X-rays, vaccinations, cancer screening, laboratory services, prescriptions, physical exams, diabetes and other chronic disease management, interpretation services, and smoking cessation services.
