Description

The Driving Under the Influence (DUI) Program offers 10 hours (5 sessions) of remedial education (Level I services) to persons with a DUI conviction who have been mandated by the court to attend substance abuse counseling. Services forDUI offenders who pose a moderate, significant, or high risk for future offenses (Levels II-III) include intensive substance abuse counseling, advocacy, and basic case management assistance.

Specialized services include orientation involving the Illinois Secretary of State’s multiple requirements for license re-instatement to include translation services, denial letters, related forms preparation, and explanations regarding necessary insurance provision.

Eligibility

-Self-pay clients only. Fees are sliding scale based on income.
-Standard program fees are as follows: low risk $200,moderate risk $400, high risk $750, very high risk $1500.

How to Apply

Cook County will conduct a DUI evaluation. After your evaluation, call your preferred Pilsen Wellness Center location to schedule an intake appointment.
You must provide:

-Photo ID
-Court Paperwork
-Registration Fee

There is not generally a wait list for services.
