Description
The Adult Mental Health Services Program serves adults, ages 18 and up, that have been psychiatrically hospitalized and discharged or diagnosed with a mental illness. These supportive services facilitate access to therapeutic and medical interventions for mental illnesses such as: bipolar disorder, depression, generalized anxiety, schizophrenia, and schizoaffective disorder, as well as disorders involving self-mutilation, anorexia, and bulimia.

How to Apply
Walk into the Center, contact them online, or call 773-564-9070 for a screening and to find out about the wait list. The wait list for other clients is generally 2-3 weeks before services begin.
The Intake Coordinator will inform you about what materials to bring. It varies depending on program (examples: SS card, insurance card, proof of income, etc.).
