Eligibility
-ACCESS accepts most Medicaid, Medicare, and private insurance plans, including Aetna, All Kids, Blue Cross Blue Shield, Cigna, CountyCare, Harmony, IlliniCare, Meridian, Molina,Next Level, and United Healthcare. Coverage is subject to change. Please contact your insurance provider to verify your benefits.
-Patients with HMO plans must have ACCESS as their primary care provider.
-All patients are welcome at ACCESS regardless of whether or not they have insurance. A staff member will be happy to help patients enroll in any benefits they are eligible to receive.
-Sliding scale fees are available to those who qualify.
-Spanish-speaking providers are available for all specialties. A Polish-speaking gynecologist and Bulgarian-speaking nurse practitioner are on staff.

How to Apply
Call 847-298-3150 and select option 2 to request an appointment, or schedule an appointment online. If you are insured, please bring the following:
-Photo I.D.
-Insurance card
If you are uninsured, please bring the following documentation to be screened for benefits eligibility or apply for sliding scale fees:
-Photo I.D.
-Proof of address (two pieces of mail or 1 piece of mail and a photo I.D. with address and name included).
-Proof of income (income tax forms, two pay stubs, SSI/SSA official documentation, two most recent pay stubs, letter from employer, or ACCESS housing/shelter or employment verification forms).
If you are seeking Suboxone treatment for an opioid use disorder,call 847-298-3150 to schedule an intake appointment. You will meet with a care coordinator and a social worker for assessment and placement in the program.
