Description

The HRDI-Child and Adolescent Outpatient Services program provides level I (outpatient) substance use treatment services for adolescents. Services include individual counseling, group therapy, and substance use education.

Eligibility

-Referrals are accepted from DCFS, the juvenile justice system, schools, and parents.
-Accepts private insurance and the following Medicaid plans: Blue Cross Community, CountyCare, Harmony, IlliniCare, Meridian, Molina, and Next Level.
-Sliding scale fees are available for uninsured clients.

How to Apply

Referrals from DCFS and the juvenile justice system are accepted but not required. Walk in to the 114th Street location during business hours for intake. Please bring the following documentation:

-Photo ID (school ID, driver's license, or state ID card)
-Insurance card, if applicable
