Description
Integrated Services is an outpatient resource for people presenting for treatment with co-occurring substance use and psychiatric disorders. Treatment for such disorders occurs simultaneously and therefore integrates substance use and mental health treatment. This Level I program focuses on abstinence from or significant reduction in use of drugs and alcohol. It also helps clients access affordable housing, primary healthcare, community-based support systems, and entitlements. IDDT also ensures compliance with court ordered treatment.
Services are offered primarily in clients' homes and community locations.

Eligibility
-For residents of Evanston, Skokie, Cicero, and Chicago (north of 115th).
-Trilogy accepts Illinois Medicaid and the following Medicaid managed care plans: Aetna, Blue Cross Blue Shield, CountyCare, Humana, IlliniCare, Meridian, Molina, Next Level, and SmartPlan Choice. Trilogy does not accept Harmony and Medicaid with a spend down.
-Trilogy participates in the First Episode program which provides funds for individuals ages 18-40 who experienced their first psychotic episode in the last 18 months. The program provides education to the individual and their family, as well as resource options.

How to Apply
Walk in during open access hours to begin the intake process. Please bring a photo ID and insurance card. Intake is available on a first come, first served basis at the following times and locations:
-Trilogy Rogers Park: Monday through Friday, 10:00am-2:00pm
-Trilogy Brighton Park: Monday 12:00pm-3:00pm
-Logan Square Neighborhood Association: Friday 10:00am-11:30am
