Description
Esperanza Health Centers provides high-quality medical care to all residents of the Southwest Side service area, regardless of their immigration status, race or ethnicity, income level or insurance status. All full-time providers and clinical staff are bilingual and bicultural. Services include:
-Physicals
-Immunizations
-Primary care
-TB testing
-Chronic disease management
-Cervical cancer screening and diagnosis
-Breast cancer screening
-HIV testing
-STI testing
-Family planning
-Pregnancy testing
-Prenatal care, including support groups and parent education
-Behavioral health consultations and referral to mental health and substance use treatment

Eligibility
-For residents of Little Village (South Lawndale), Pilsen (Lower West Side), Brighton Park, Archer Heights, Gage Park, Chicago Lawn and the suburb of Cicero.
-Esperanza accepts most insurance plans, including the following Medicaid Managed Care plans: BCBS, County Care, Harmony, IlliniCare, and Meridian.
-Sliding scale fees are available for those without insurance. Fees range from $30-$85 and include the cost of lab testing.
-Esperanza participates in the Vaccines For Children (VFC) program. If your child is 1) less than 19 years old and 2) uninsured, Medicaid-eligible, an American Indian or Alaska Native, or underinsured, they are eligible for free vaccines through the VFC program. Click here for more information.

How to Apply
Call 773-584-6200 to learn more or schedule an appointment. Intake staff will collect basic personal information over the phone and connect you to the appropriate staff or clinic. Phone intake is available Monday through Friday, 7:30am-6:30pm.
