Description
UI Health's Community Reintegration program is appropriate for adults leaving inpatient treatment, as well as those who can benefit from more intensive services but do not require inpatient treatment. Primary Coordinators work with clients, families, and outside providers to create a treatment plan and discharge plan. Program length ranges from a few weeks to a few months. Group-based services include Cognitive Behavioral Therapy (CBT), communication skills and assertiveness, games and leisure skills, mindfulness, health education, stress management, medication and diagnosis education, recovery skills, and symptom management. Case management, counseling, and family support are also provided.

Eligibility
-Clients must have an outpatient therapist or psychiatrist.
-The program does not include substance use services. Clients with a substance use disorder are assessed and admitted on a case by case basis.
-UI Health accepts Medicaid managed care Blue Cross Community Health and Meridian plans, as well as the following MMAI (dual eligible) plans: Aetna Better Health, Blue Cross Community MMAI, IlliniCare, and Meridian.
-UI Health accepts the following marketplace insurance plans: Blue Choice Preferred PPO, Blue Cross Blue Shield, Blue Precision HMO, Humana HMOx,

How to Apply
You must be referred by a mental health professional. Referrals can be made by calling 312-996-1065, or by submitting a completed referral form via fax (312-996-9517) or email (amagana@psych.uic.edu or adrianm@psych.uic.edu). You will be contacted to schedule an intake assessment, generally within a few days of the referral.
