Description
Branden House is an 80 bed Level III.5 treatment facility for adults with substance use disorders. Services include short term group counseling, service planning, connection to other needed resources (including health care), transportation assistance, substance use education. and on site HIV testing and counseling. Methadone maintenance is available as needed. Branden House is a 28 day program, though the stay may be extended for a total of up to 90 days.

Eligibility
-For residents of Cook, Kane, Kendall, and Will counties.
-Target populations include: pregnant and postpartum women, parents, people living with HIV, people involved with the criminal justice system, and those with a dual diagnosis.
-SASI accepts Blue Cross Blue Shield, County Care, Harmony, IlliniCare, Meridian, Molina, and Next Level Health. There is a co-pay of $70 per month to cover the cost of food.
-Self pay clients are accepted. The fee is $3200, with $1600 due within 10 days of admission.

How to Apply
Walk ins are accepted, but calling ahead is preferred. Call 312-808-3210 x1 or walk in during business hours for an eligibility screening and to schedule an assessment. Intake may be done at either Branden House or the SASI office. Your insurance benefits will be verified prior to admission. You must bring the following documentation:
-Photo ID (driver's license or state ID)
-Proof of Social Security number (Social Security card or printout from the Social Security office
-Insurance card or medical card, if applicable
-Award letter or copy of grant check for SSI, SNAP, TANF, unemployment, or other government assistance, if applicable
-Proof of income (check stub, W-2 form, or employer letter)
-Verification of opiate addiction by another adult
-Medical verification of pregnancy and prenatal care, if applicable
Clients are generally able to be admitted within a week after intake.
