Description
Hamdard provides integrated outpatient behavioral health care with an emphasis on prevention and early intervention. Services are provided by licensed counselors, social workers, clinical psychologists, and psychiatrists. Issues commonly addressed in counseling include:
-Acculturation difficulty
-Adjustment difficulty at work or school
-Anxiety disorders, including social anxiety, panic disorder, and obsessive-compulsive disorder
-Depression
-Family conflice
-Marital troubles
-Post-Traumatic Stress Disorder (PTSD)
-Substance use disorder (mild to moderate)
Outpatient psychiatric care and medication management are available if needed. Hamdard also provides assessments and evaluations for disability benefits and Social Security Income (SSI).

Eligibility
-Psychiatry services are available only to patients with a primary care provider at Hamdard. You must meet with a Hamdard counselor for at least one session before receiving psychiatry services.
-Hamdard accepts most private insurance, as well as Medicaid and Medicare plans. Patients with HMO plans must have Hamdard as their primary care provider.

Payment
How to Apply
Contact your preferred location directly for an eligibility screening and insurance verification. If eligible, you will be scheduled for an initial appointment to review your mental health history and develop a treatment plan. Please bring the following to your appointment:
-Photo ID
-Insurance card
-Current proof of address (e.g., utility bill, lease)
-Proof of income (e.g., pay stubs, W-2, employer letter, SSI/SSDI/unemployment benefits statement)
-List of all current medications
-List of allergies, if applicable
-Social Security number (requested, but not required)
A wait list for counseling may apply: there is generally a wait list for psychiatry.
Counseling hours are as follows:
-Addison Office: Monday through Friday 9:00am-5:00pm
-Chicago Office: Monday, Tuesday, Thursday, and Friday 9:00am-5:00pm, Wednesday 12:00pm-8:00pm
Psychiatry hours are as follows:
-Addison Office: Tuesday and Friday 9:00am-5:00pm
-Chicago Office: Monday, Thursday, and Friday 9:00am-5:00pm, Tuesday 11:00am-4:00pm, Wednesday 12:00pm-8:00pm, Saturday by appointment
