Description
American Indian Health Service of Chicago, Inc. offers a full spectrum of mental health and substance use services for individuals of all ages.. Services include:
-Mental health counseling for individuals, families, couples, and groups
-Group and individual substance use counseling
-Methamphetamine and Suicide Prevention Initiative (MSPI), including counseling, group therapy, and Youth Mental Health First Aid training
-Domestic Violence Prevention Initiative (DVPI), providing counseling and case management for survivors and prevention programs for youth
AIHS also hosts weekly AA meetings.

Eligibility
-You do not need to be a patient of the health clinic to receive behavioral health services.
-American Indian Health Service of Chicago accepts a variety of Medicaid, Medicare, and private insurance plans, including Aetna, Blue Cross Blue Shield, Cigna, CountyCare, Humana, IlliniCare, Meridian, Molina, and United Healthcare.
-Sliding scale fees based on income are available for uninsured clients.
-The program coordinator speaks some Spanish, but is not fluent.

How to Apply
Call 773-883-9100 and ask for the intake coordinator to schedule an intake appointment. The intake coordinator is only available part time, and intake hours vary. If no one is available, leave a message with your name and number and your call will be returned as soon as possible.
Please bring the following documentation to your appointment:
-Photo ID
-Tribal enrollment card
-Insurance card,if applicable
-Proof of income, if applying for sliding scale fees
You will also be asked to complete new patient forms, including patient information and medical history.
