Description
Catholic Charities Adoption/Subsidized Guardianship Preservation Services, a program of the Family First Initiative, supports adoptive and guardianship families throughout Cook County through challenges related to adoption and guardianship issues. Services are intended to maintain family stability and promote an understanding of how the adoption/guardianship story impacts clients' lives. Available services include:
-Crisis Intervention
-Individual and family counseling
-Family and child support groups
-Advocacy with schools and other agencies for specialized services
Services are provided in clients' homes. Evening appointments are available.

Eligibility
-Adoptive families may have adopted through DCFS or via domestic, private, or international adoption.
-Services for guardianship are available only to families with legal guardianship through DCFS. Catholic Charities is unable to serve families who have legal guardianship through probate court with no DCFS involvement.
-There is no cost to clients for services. However, Catholic Charities will bill Medicaid if available.

How to Apply
Call the intake line at 312-655-8357 for a brief intake interview. The interview will include collection of demographic information and a discussion of your reason for seeking services.
