How to Apply
Call 312-655-7725 to schedule an intake assessment. The intake specialist will help a client determine eligibility for health insurance coverage and will complete a 12 item questionnaire in order to collect basic information. The client's case is then referred to the ACES Director who will determine which level of service the client needs: either Level 1 Outpatient or Level 2.1 Intensive Outpatient therapies. A counselor is then assigned to client.
Can be referred through an inpatient program or by self-referral.
